This is a CUPE / UBC horror story that changed dramatically in DECEMBER 2019. When I found PHOTO ID of UBC Forensic Psychiatrist Dr Elizabeth Zoffmann. What I was looking at in December 2019 on top of FORGERY was IDENITITY THEFT... I will get to this later. However this IDENTITY THEFT is REASON WHY The College Of Physicians and Surgeons LIED and threw myself and family under the bus. This by full approval of ALL CONTENT in TWO Differently Formatted versions of The Worlds Worst Forensic FORGERY.

However Dr. Zoffmann as the Affiant does NOT authenticate it by her Signature... Her sworn AFFADAVIT is signed ONLY by Dr. Robert Vroom from The British Columbia College Of Physicians and Surgeons.
Why did Dr. Elizabeth Zoffmann the Affiant not sign her sworn AFFADAVIT ? Perhaps also a Fraudulent document... Certainly not Legal.
Due to the the Combined POWERS of UBC , The CUPE National with support from the British Columbia Medical System. This known to be by ALL Forensic Forgery going into 2025... REMAINS OFFICAL and STANDING as my life long Forensic Psychiatric Medical / Criminal Record. This comes with a HEREDITARY FACTOR which is ETERNAL.
The HEREDITARY FACTOR of what was FALSELY documented against ME Mental Illness wise in 2022 was USED against a family member. This preventing a very serious Spinal Surgery. See College GASLIGHTING Letter from August 2008, the same family Doctor De Marchi who also supported the Zoffmann Forensic Forgery and fully supported ME as being Delusional and Mentally Ill, struck again in 2022 against a family member with more LIES of dillusion using a Family History of MENTAL ILLNESS.
Why did The British Columbia College Of Physicians and Surgeons in 2007, 2008 and 2009 who were fully aware of FORGERY of a FORENSIC Psychiatric Medical / Criminal Report throw myself and family Lineage under the bus?
I believe they were also FULLY AWARE of the IDENTITY FRAUD. From 2005 to 2011 The Business Manager of The British Columbia Hospital Employee's Union was Judy Darcy. Darcy was The CUPE National President in 2001, and the person who I BELIEVE actually PERFORMED this Forensic Psychiatric Medical / Criminal Evaluation on ME on May 25, 2001.
This was a VIOLATION of myself and my Family Members Privacy by a NON MEDICAL CUPE National President. Also this FORGERY passed around CUPE National, CUPE Local 116 Executive and UBC / HR Personnel, before even I had the opportunity to review it.
ALL Doctors are fully aware that in accordance to the Law a Legal Medical Document cannot be ALTERED by anyone other than the Doctor who wrote it. Even then by the autoring Doctor altered only under exceptional circumstances.
This was no ordinary Medical Document, it was a FORENSIC Psychiatric Medical / Criminal Report.
A FORENSIC Psychiatric Medical / Criminal Report where a Canadian Citizen is documented as very narrowly escaping being committed under the Mental Health Act... As in Borderline Insane and highly suspected to be in the possession of FIRE ARMS, which is repeated over and over. This type of document is FILED under the Jurisdictions of BOTH the Judicial System and the Medical System. Under NO circumstance's can anything be REMOVED from or INSERTED into this type of Document. It is probably one of the most Prestigious Court Regulated Documents in the Land.
NO error, no additions or submissions, no bias, no duplications are permitted by Law. Also if the objective is Criminal Dentention or Criminal Sanctioning under the Mental Health Act. Also fair representation must by Canadian law be given.
On May 25, 2001 allegedly a Forensic Psychiatrist by the name of Dr. Elizabeth Zoffmann performed a Forensic Psychiatric Medical / Criminal Assessment / Evaluation on me who was John Stewart.
Firstly... Why would I undergo this type of FORENSIC Medical / Criminal Evaluation?
Secondly... Why would I NOT be permitted any WITNESSES to attest to my side of the story. See Dr. Zoffmann's (unsigned) AFFADAVIT below.
Thirdly... Why would the Forensic Psychiatrist Client be witnessed Coaching on an expected result, in an Assignment to see if I meet the Criteria for Certification Under The Mental Health Act.
Fourthly... Why would a critical life determining procedure which include a CT Scan of the HEAD / BRAIN be HIDDEN and BLOCKED by the Forensic Psychiatrists Client.
This would be the type of Evaluation given to perhaps a Serial Killer, or some type of Psychopath.
As per my MAIN STORY, I had sent ONE RUDE E-MAIL with a TYPO to my CUPE local 116 Executive, this in RETALIATION for years of Oppression. The CUPE NORM at UBC for a Member who momentarily loses it was an Anger Management Consultation.
Outside of UBC the closest thing to a Criminal Record was ONE Photo Radar Ticket.
Again WTF, why was there some type of ASSINGMENT given by the CLIENT ( CUPE National) to have me Committed under The Mental Health Act?
Why would a Forensic Psychiatric Medical Expert known by the Criminal Justice System as an EXPERT Medical Witness implicate her CLIENT by giving an empathetic APOLOGY for FAILING ?
I John Stewart was at the age of 41 on March 22, 1989 I began my employment as a UBC Steam Fitter. I was firstly a UBC Member and Secondly a CUPE Local 116 Member.
I was originally from Glasgow Scotland, immigrating to Canada on September 1976 at the age of 28. In the UK I severed as a 5 year Apprentice in the Aircraft Industry, moving from that industry to Sheetmetal and then Pipe Fabrication. Also in the Scotland I was 10 Year Reservist in British Airborne.
In Canada from September 1976 until joining UBC in March 1989 I had a 15 year Career in Planning / Supervision in the Shipyards.
Since coming to Canada I was never a Hunter or even a Fisher , I can honestly say to this day in Canada I have never even touched a LIVE Fire Arm.
Getting back to UBC Dr. Elizabeth Zoffmann and my alleged Forensic Psychiatric Medical / Criminal Evaluation on May 25, 2001.
By Canadian Law the Medical Content from any type of Medical Document first an foremost belongs to the Patient or potential Prisoner / Victim. On request by Law it cannot be refused by the Doctor or Client. 30 days is I believe the Maximum holding time.
From my main story I believe my first request for my Medical Information was April 22 2002 or there abouts. It was REFUSED.
Dr. Elizabeth Zoffmann had returned her alleged Forensic Psychiatric Medical / Criminal Report back to her Client on June 12, 2001.
Who was UBC Dr. Elizabeth Zoffmann's Client? It was NOT UBC / HR they were NOT witnessed on board until July 2008.
As per my MAIN STORY, UBC / HR or UBC Equity totally refused my plead for HELP against my UBC Oppressors who were my CUPE Local 116 Executive.
On March 03, 2006 I made an OFFICIAL Request to the UBC / AVP Lisa Castle to review the Document as written by Dr. Elizabeth Zoffmann. Lisa Castle I believe was also HEAD of UBC / HR. It was not forthcoming in the 30 days, as in my request was IGNORED.
As per my MAIN STORY by the early summer of 2006 I had a Lawyer on board, a Tom B. His one job was to retrieve what was MY Medical Information... He took my money however, he FAILED.
On December 08, 2006 The CUPE National's Dianne Jolly under the nose of this Lawyer would be the first person to release a COPY of UBC Dr. Elizabeth Zoffmann's Report. It had ILLEGAL Editing out (Tampered with) so it was a FORGERY ... however my Lawyer Tom B never called it. WHY? On hind sight, I was set up with this Lawyer, from within UBC.
December 08, 2006 was 5 1/2 YEARS after Dr. Zoffmann's delivered her report to her CLIENT on June 12, 2001.
With my full knowledge of FORGERY, for the sake of peace, by around late February 2007 I had AGREED with my Lawyer Tom B and The CUPE National's Dianne Jolly to GIVE UP MY QUEST for UBC Dr. Elizabeth Zoffmann's OFFICIAL Report.
A NEW BEGINING was now AGREED on with my Lawyer Tom B and The CUPE National's Dianne Jolly. At this point I was SO GLAD IT WAS FINALY OVER.
What happens next is in my MAIN STORY. A UBC Manager TIP OFF where he simply says BEWARE JOHN. Was I being SET UP, but the content on being documented as Borderline Insane and the Continuos REPEATING of FIRE ARMS yet to found by COLLATERAL MEAN'S. .. Well lets say ALLERTED ME SOMEWHAT.
From around late March 2007 my quest for the TRUE OFFICIAL VERSION of UBC Dr. Elizabeth Zoffmann's Document is now back on. As in I required a Report that would identify NO ILLEGAL EDIDITING out. From wittnessed EDIDING OUT of CONTENT, opens the door to EDITING IN of CONTENT... As in FIRE ARMS yet to found by COLLATERAL MEAN'S, again repeated over and over.
Late March or Early April 2007 is about the time frame I take this FORGED Version of The Zoffmann Report to the British Columbia College Of Physicians and Surgeons. As per letter Below Dated August 2007 it is fully supported by THREE British Columbia Doctors. Dr. Elizabeth Zoffmann, Dr. Ronald De Marchi and the person from the College Of Physicians and Surgeons B. T. B. Taylor.
NOTE: Get this COOINCIDENCE. My Lawyer Tom B. who I was highly suspicious of, hired by ME to get the Zoffmann document... In April 2007 in the EXACT SAME WEEK, both my lawyer Tom B and Dr. Elizabeth Zoffmann now come under British Columbia Ministerial Orders.
Further Proof of FORGERY comes in July 11,2008 when The CUPE National's Dianne Jolly release's a SECOND Differently Formatted Version of Dr. Elizabeth Zoffmann's Forensic Psychiatric Medical / Criminal Report on ME.
For my REFUSAL to BELIEVE the CUPE National terminate my 19 Year CUPE Membership, as in sever my families livelihood.
In July 2008 UBC/ HR are for the very first time WITNESED on board, this with UBC / HR'S Lindi Frost. Frost BLOCKS my attempt to return to my employment as a UBC Steam Fitter with a threat of RCMP INTERVENTION.
UBC / HR'S Lindi Frost is now SOLELY pushing what is to be my THIRD Psychiatric Evaluation at UBC. From July 2008 she has named UBC Dr.Alan Buchanan to perform this.
In July 2008 I hand deliver TWO Differently Formatted Versions of Dr.Elizabeth Zoffmann's Forensic Psychiatric Medical/Criminal Report to the Office of Dr.Elizabeth Zoffmann and The British Columbia College Of Physicians and Surgeons.
Dr. Elizabeth Zoffmann does NOT Respond. Dr. Vrooms response as below in his GASLIGHTING Letter... "TWO differently formatted versions are EXACTLY the same"... he now considers this matter CLOSED.
By August 2008 due to The British Columbia College Of Physicians and Surgeons Dr. Robert Vroom my families livelihood has been severed. I am now STUCK with a FORENSIC Psychiatric Medical / Criminal Record ... How can this be CLOSED.
I am now back at OPIC trying to force UBC to OFFICIALLY release a copy of Dr. Elizabeth Zoffmann's Report... I am honestly expect a different type of Report... something closer to an Anger Management Report.
On January 16, 2009 the UBC Freedom Of Information Coordinator Christina Ulveteg FINALLY release's the Zoffmann Report. However it is the EXACT SAME DOCUMENT that the CUPE Nationals Dianne Jolly had released on July 11 2008. A FORENSIC FORGERY.

LINK. What is the PURPOSE of the GAF SCORE.
The GAF is based on a scale that was first used in 1962. It's been updated over time. In 2013, the manual that psychiatrists in the U.S. use to define and classify mental disorders dropped it in favor of a scale designed by the World Health Organization.
So it changed in 2013, however compulsory prior to 2013 as in 2001 and 2009 . The YEARS of my alleged FIRST and THIRD Psychiatric Evaluations by UBC Psychiatrists.
The GAF SCORE would be the VERY FIRST THING a cross examining Medical OR Criminal Expert would REVIEW. In layman's term's REQUIRED to MONITOR Regress and Progress of a Mentally Ill Patient or Patient / Criminal.
In my case where it became clear I was documented as verging on Criminally Insane and a thought threat to Society. The GAF SCORE had to EXIST as BELOW 20. However it DID NOT EXIST.
ALL MEDICAL FOLKS WHO SUPPORTED FORGERY HAD TO BE AWARE OF THE MISSING AXIS 1V AND AXIS V WHICH WAS THE GAF SCORE.
IN LATE 2009 THE INCOMPLETE DIFFERENTIAL DIAGNOSIS WITH THE OMISSION OF THE GAF WOULD BECOME MY MEDICAL PROOF OF FORGERY.

My NAME was spelled wrong 4 times... Steward instead of Stewart.
The DATE of the Evaluation was WRONG as in Sunday March, 25, 2001 when it was Friday May 25, 2001.
There was some simple MATH calculations that were WRONG.
There was 2006 content in the 2001 Document. This about IRA graffiti on walk path near my house.. written by kids in early 2006, it remained there until at least 2008... This content should not have existed in a 2001 Forensic Psychiatric Medical/Criminal Report.. Mentioned twice by the clairvoyant Dr. Elizabeth Zoffmann
While it was documented that there was NO Past history of violence. However there was 3 TIME'S REPEATED content on FIRE ARM'S that had yet be found by COLLATERAL MEAN'S... Another future event, yet to happen.
In her OPIC File No. F08 34272 AFFADAVIT documents that UBC did NOT PERMIT her to interview any UBC Co- Workers. This would indicate that NO WITTNESSES where ALLOWED for ME to confirm or contest my side of the story. Based on NO Past History of Violence... It is clear her CLIENT have gave Dr. Zoffmann some kind of INSTRUCTION or ASSINGMENT to see if I meet the Criteria for Certification Under The Mental Health Act. By LAW this would be HIGHLY ILLEGAL an ASSINGMENT coming from a THIRD party that I have never met.
From this UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann gives an Empathetic Apology for FAILING. "UNFORTUNATLY Not quite meet the criteria"... she documents. From this I am documented as Borderline Insane, so still very seriously ill. Also very likely to be in the possession of FIRE ARM'S as above repeated OVER an OVER.
It is very clearly documented that The UBC Medical Expert Dr. Elizabeth Zoffmann see's me a very seriously Mentally Ill. She recommends to her CLIENT (Who had given her an ILLGAL Assignment in regard to locking ME up in an Asylum) that a Full Neurological Examination including a CT SCAN of the HEAD / BRAIN be given to rule out any physical condition that could be causing my Psychiatric Presentation. WOW. If all is MORAL this is ASAP a RED FLAG must be raised. IMEDIATE MEDICAL ATTENTION REQUIRED.
From UBC Forensic Psychiatrist's Forensic Psychiatric Report on Me... We are certainly witnessing conspiracy to FIRST... with NO PAST History of Violence LOCK ME UP IN AN ASYLUM. Now comes the most UNBELIEVABLE PART. What did UBC Forensic Medical Expert SUSPECT... was it a Blood Clot... was it a Brain Aneurism...was it a Cancerous Tumor or perhaps a Cerebral Hemorrhage. OPIC File No. F08 34272 Dated August 31, 2009. Which is UBC Forensic Psychiatrist Dr. Elizabeth Zoffmann's SWORN AFFADAVIT confirming she first heard of this Shortly before May 18, 2001... As the AFFIANT where it is required my law she MUST sign. However she does NOT SIGN.
I knew about it Mid April 2001, actually April 10th 2001 I had a letter . She terms the Evaluation as a Risk Assessment and NOT a FORENSIC Psychiatric Medical/ Criminal Evaluation which she actually dates she performed on March 25, 2001 which was a SUNDAY on a guy named John Steward who she mentions way more than ME. These inaccuracies have to be noted as NOT Forensic.

The Report sent by ME to The College Of Physicians and Surgeons was issued by The CUPE National's Dianne Jolly on December 08, 2006. I referred to it as VERSION 1 Of Dr.Elizabeth Elizabeth Zoffmann's Forensic Psychiatric Medical / Criminal Report . This from a Forensic Psychiatric Medical / Criminal Evaluation allegedly performed by UBC Forensic Psychiatrist Dr.Elizabeth Zoffmann. From December 2019 my BELIEF actually performed by The CUPE National President Judy Darcy.
A LEGAL DOCUMENT of any description cannot identify EDITING OUT. More so a FORENSIC Psychiatric Medical / Criminal Report where the INTENT was to put a Canadian Citizen into an ASYLUM. This type of Report falls under BOTH the Medical and Criminal Justice Systems.
